About the role
POSITION SUMMARY Under the direction of the Manager of Housing Based Support Services, the ICMS Case Manager will be responsible for providing coordinated Case Management and Supportive Services for formerly homeless individuals diagnosed with special needs. The Case Manager will deliver services on-site to tenants who reside at a newly developed multi-unit permanent supportive housing complex.
ESSENTIAL DUTIES & RESPONSIBILITIES
- Work with each tenant to conduct an initial housing intake and needs assessment
- Assist tenants in determining personal goals and developing Individual Service Plans (ISP)
- Conduct a minimum of two face-to-face contacts with each tenant monthly
- Conduct quarterly meetings to discuss tenant progress and accomplishments
- Act as liaison among Property Manager, property management company, tenant, and primary mental health provider
- Provide information regarding Fair Housing, tenant rights and responsibilities, rental agreements and property rules
- Coordinate linkage to services including life skills workshops, money management, educational attainment, health services, counseling, job training and placement
- Assist clients in obtaining rental assistance and eviction prevention services
- Address tenant grievances, mediate disputes, and resolve issues affecting tenancy
- Develop workshop opportunities on relevant topics including housekeeping, money management, parenting, daily living skills
- Maintain data collection in DHS CHAMP database and prepare progress reports
- Monitor status of tenants and prepare those ready to transition to mainstream housing
REQUIREMENTS
- 1-2 years of experience or related work and BA Degree
- Experience working with youth experiencing homelessness and housing instability, and/or experience in foster care, probation, or mental health systems
- Reliable, flexible team player with good work ethic and ability to maintain personal boundaries
- Valid driver's license, car insurance, and reliable transportation
- Proficient in Microsoft Office suite
- This is not a work from home position; role reports to office 5 days a week
