Social Work Case Manager

Social Work Case Manager Salary Range: 26.04 to 31.24 Hourly Location: Escondido, CA The Social Work Case Manager is responsible for providing case management services to participants intended to promote self-sufficiency according to the participants' identified needs and the programs' focus. The Social Work Case Manager supports participants through a human centered and trauma informed approach, in addition they are engaging, culturally responsive and aware of the diverse needs of various cultures, races, ethnicities and populations.

SCHEDULE:

• This position is Monday – Friday, 8:00AM – 5:00PM with evening and weekend availability based on client and program needs

WORK DESIGNATION:

• In-Person at our Escondido residential facility
• Travel required throughout San Diego County to meet with clients in the community or in their residences
• Please note that the work designation may change based on business needs.

PROGRAM SUMMARY:

Our program provides transitional housing and supportive services to former foster youth ages 18–25 as they work toward greater independence and long-term stability. Youth live independently in housing provided through our program, with placements available at our single-site location in Escondido as well as in community-based apartments throughout the area.

The goal of the program is to provide safe, stable housing while supporting each young adult in developing the skills and connections needed to successfully transition into independent adulthood. Along with housing, youth receive individualized support focused on relational wellness, independent living skills, employment and career development, education, mental health and overall well-being.
