Amity Foundation, an internationally acclaimed Teaching and Therapeutic Community, is seeking a compassionate and enthusiastic ECM Program Coordinator to work with prison and re-entry programs.

About the Position: The ECM Program Manager oversees the Enhanced Care Management (ECM) - Community Supports (CS) project in the assigned County. Supports the Management Team and oversees day-to-day administration of programming activities and provides direct supervision to faculty working on this project. ECM is a comprehensive, patient-centered approach to healthcare that aims to improve outcomes for high-need populations, involving coordination of medical, behavioral, and social services to address the full range of patient needs, focusing on those with complex and chronic conditions.
Key Responsibilities:
- Oversee development, implementation, and continuous improvement of ECM-CS program in assigned county
- Manage and supervise assigned staff/team
- Comply with contract requirements and state/federal regulations
- Train multi-disciplinary team (Advocates, Housing Navigator, Quality Assurance Specialist)
- Work closely with team to identify necessary clinical and non-clinical resources
- Foster collaborative culture encouraging teamwork, continuous learning, and excellent care
- Establish and maintain relationships with healthcare providers and community organizations
- Provide oversight to billing/claiming processes
- Evaluate program operations for effectiveness and compliance
- Ensure compliance with Amity policies and applicable contract obligations
- Attend community building functions and training workshops
Qualifications:
- Excellent interpersonal and communication skills
- Knowledge of contract parameters and contracting processes
- Understanding of local, state, and federal regulations
- Ability to integrate budgetary and service utilization data
- Strong multi-tasking and problem-solving abilities
- Ability to work constructively with diverse people
- Proficiency with Microsoft Office Suite
- Knowledge of Amity policies and procedures
