We're looking for someone to join our team as a AFS Case Manager (PAT) who:

The PAT case manager works with families referred by the Kern County Department of Human Services to increase child safety through improving family stability and well-being. The PAT case manager facilitates a 2-year home-visiting parent education curriculum, conducts developmental assessments, links families with community resources, and encourages them to proactively engage in preventing the need for further involvement with Child Protective Services.

Essential Functions:

  1. Responds to assigned referrals to engage families in case management
  2. Conducts intakes for program enrollment, assessments, program releases and confidentiality agreements
  3. Conducts in-home parent education visits and regular communication with PAT families
  4. Engages PAT families in the program using a trauma-informed approach to encourage growth and success
  5. Provides group connections for PAT families to encourage peer learning and support and foster connectedness
  6. Advocates for clients when there is a concern with the service delivery system
  7. Maintains assigned case load as required by Scope of Work, contract requirements and program protocols
  8. Maintains accurate, detailed, and up-to-date records including contact summaries, progress notes, support service plans, and program specific reports
  9. Represents the PAT program in local collaboratives and other settings
  10. Serves as liaison with other community agencies and schools
  11. Reasonable and predictable in person attendance.
  12. Other duties as assigned

You'll be successful with the following qualifications:

  1. B.A./B.S. in a health, social science or related field is strongly preferred. Comparable years of experience in a similar field may be substituted
  2. Experience working with diverse target populations, including those who have experienced trauma
  3. Familiarity with community resources
  4. Ability to maintain client dignity and confidentiality at all times
  5. Bilingual (Spanish-English) strongly preferred
  6. Flexibility to alter the 8:00-5:00 schedule when necessary to meet client needs
  7. Excellent written and verbal communication skills
